Joining our established team you'll be providing a social work service to a highly professional standard. The Hospital Social Care Teams work with all adult customer groups within Social Care & Support to assessing their needs and arranging support to meet Care Act eligible needs in partnership with health service staff.
Assessments are completed in all hospitals and other community settings to increase choice and control through personalisation, supported self-assessment and self-directed funding.
The teams have a mixture of staff skill sets and roles that include:
Social Workers Social Care Workers Admin Age UK Social Subscribers Trusted Assessors Reablement Assessors
In addition, you'll be communicating with a wide variety of clients, communities, partners and agencies as well as working on your own initiative, often in pressurised situations
Team members are supported by their colleagues and Team Leaders who deliver regular supervision and day to day support.
Staff also have access to regular training provided by Warwickshire County Council
The Hospital Social Care Service is based across a number of hospital sites in Warwickshire. The teams work very closely with our Health colleagues to facilitate timely safe, supportive and suitable discharges for our customers. It is an exciting time to join our team as we together look at innovative ways of delivering a 7 day service. There will be an expectation of working 1 in 4 weekends to provide managerial support to the teams.

Key Requirements
To be successful in this role it's important you have a professional Social Work qualification and are registered with the Health and Care Professions Council.
It is also IMPORTANT AND ESSENTIAL to have a clean driving licence for your role as a Social Worker.
It's also important you have strong analytical and communication skills both written and verbal and can work on your own initiative as well as collaboratively with individuals, families, carers, communities, colleagues and other agencies. With the ability to work to tight deadlines, often under pressure, whilst at all times maintaining composure and your professional stance, you will be used to making difficult decisions. With strong influencing, persuading and resilience skills you'll also have child care experience and good IT skills.
Applicants should have experience of working with adults with care and support needs.
