The Leaving Care Team is looking to recruit a Personal Advisor.
The role of the Personal Advisor is to support care leavers transition towards independence. You will work with young people from 18 - 25 who are identified and support them to develop a Pathway Plan assessing need and identifying appropriate support. You'll be liaising with a wide variety of other professionals in housing departments, accommodation providers, Job Centre, education or training providers as well as liaising with families to build up local networks. Care leavers can face many uncertainties as they make the transition from foster care to independence and it is crucial that their support needs are met to provide them with stability and security into adulthood.
