About Us
Do you fancy trying a new approach to working with older people within a hospital acute setting?
The Hospital Social Care team is currently pioneering a new approach to front line working which focuses on customer strengths and promoting use of universal services.
You will be required to work with a variety of customers from 18 years and above who come into our acute and community hospitals. These include people with learning disabilities, physical disabilities, mental health, carers and their families and other professionals.
You will be required to undertake outcome focused assessments, support planning and reviews. Focusing on strengths, you will enable customers to identify outcomes, meet their full potential and maximise their independence. As a strong team player, you will be required to contribute to the development and promotion of the Hospital Teams, using imaginative, creative and innovative ways of meeting the support needs of adults and their carers.
The roles require applicants to be able to work under pressure within hospital setting and discharge customers promptly.
We have three hospital sites across Warwickshire County Council/Coventry. We are dedicated and supportive teams. We aim to respond quickly and efficiently in offering strengths-based assessments & providing support to customers and their carers within fast paced environment.

About The Role
Permanent Level 3 Social Worker (Senior Practitioner)- George Eliot Hospital A & E and Acute Wards– to support quick turnaround of customers from A & E. You will also be supporting with discharges from Acute Wards.
Permanent Level 3 Social Worker (Senior Practitioner) – D2A South of Warwickshire Team -Mainly Attending Decision Support Tool meeting, raise disputes, attend multi-disciplinary meeting. Care Act assessment, support plans, reviews and anything else in line with job description.
Our teams also operates county wide and has bases in South of the County- Warwick, North Warwickshire, Rugby and Bedworth and teams comprises of Social Care Practitioners and Social Workers.
Joining our established team, you'll be providing a social work service to a highly professional standard. Managing a caseload and travelling throughout the locality of North and South of Warwickshire depending with business need, you'll be working with older adults and their families and unpaid carers.
Undertaking and recording robust assessments and care and support planning will also be part of your role.
In addition, you'll be communicating with a wide variety of customers, communities, health partners and agencies as well as working on your own initiative, often in pressurised situations.
Team members are supported by their colleagues and Team Leaders who deliver regular supervision and day to day support. Staff also have access to regular training provided by Warwickshire County Council.
